New Patient Paperwork
Thank you for choosing our office to be your dental home. You can print off our office's new patient paperwork to fill out at home and bring with you to your appointment. You can also arrive 15 minutes prior to your appointment to fill it out at our office.
Dr. Coen offers our in-office Care Plan to patients without dental insurance. It is $150 a year for all your Exam and X-rays. A cleaning is $80 as long as you do not have Periodontal disease. With the plan you get 20% off dental services (some limitations).
Existing patients can print and fill this out at home to bring with them to their appointment. You can also arrive 15 minutes prior to your appointment to fill it out at our office.
